First, you got to scan the form and save it as a jpg file.  Then, paste the jpg file onto a graphics software, in this case, I use Coreldraw.  Just type the answers (text) on the pasted form and save the file.
When you want to print, remove/delete the pasted jpg form leaving only the typed text on a blank page and print it through an inkjet printer with the original form in the paper feeder.
