FOR A FULLY SCULPTED FIGURE PROMOTION
TERMS & CONDITIONS
1. Information on how to enter and prizes form part of these conditions. By participating,
entrants agree to be bound by these conditions. Entries must comply with these conditions
to be valid.
2. Entry is open to permanent residents of Australia, aged 16 years and over. Employees of
the Promoter and their associated agencies and companies and their immediate families
are ineligible to enter.
3. To enter, simply purchase any tub of Black Swan branded Greek Style yoghurts (375g or
500g) with the promotional tag, at any participating Woolworth’s stores located within
Australia, during the Promotional Period and retain your receipt. Then to register either;
• Log onto www.fullyhealthy.com.au , follow the prompts to the promotional entry
page and enter your name, address, daytime phone number, email address and the
last four (4) digits of the barcode of your favourite Black Swan Greek style yoghurt
(375g or 500g) during the promotional period; or
• Mail in the completed entry form on the entry pad with a clear photocopy of your
receipt to “c/o “BLACK SWAN FULLY SCULPTED PROMOTION” P.O. BOX 186
PRAHRAN POST SHOP, PRAHRAN VIC 3181 during the promotional period;
4. Entrants may enter as many times as they like but only one entry per valid purchase of
each Woolworths Black Swan Greek style yoghurt (375g or 500g) is permitted. Entrants
must retain their original receipt or a clear photocopy of their original receipt as proof of
valid purchase during the promotional period.
5. Offer is not valid in conjunction with any other special, discount or promotion offer unless
indicated otherwise.
6. The Promoter reserves the right to request verification of identity, residential address of
winners at the nominated prize delivery address and/or proof of entry validity i.e. proof of
purchase (including store receipt and product barcode for purchase requirement) and other
information relevant to entry into or participation in this promotion. Verification is at the
Promoter’s discretion, whose decision is final. The Promoter reserves the right to disqualify
any individual who is involved in any way with interfering or tampering with the conduct of
the promotion.
7. The promotion commences at 12.01am AEDST on 15/11/10 and finishes at 11.59pm
AEDST on 15/12/10 (“Promotional Period”). Entries close last mail received 11.59pm
AEDST on 15/12/10.
8. The draw will take place at Level 3, 554 Church Street Richmond VIC 3195, Australia, on
21/12/10 at 11.59 am AEDST. All valid entries received within the Promotional Period will
be entered into the prize draw.

9. There will be 20 Major prizes in total. Each prize will consist of 1x Fitness First Platinum
Annual (12 month) Membership. Value is $1,400.00 each. 1 x Results Pack from Fitness
First (including 3 x 45 minute Personal Training Sessions, Results Journal, Comprehensive
Nutritional Guide & Results Guide) valued at $74.95 each and 1 x Woolworths Wish Gift
Card valued at $550.00. Total individual prize pack is valued at $2,024.95 each.
10. All competition entrants who enter the promotion via Online or Mail as set out in Clause 3
above will each receive a complimentary 3-day Guest Pass to Fitness First. To download
the complimentary Fitness First Guest Pass, entrants must log on and go to
www.fitnessfirst.com.au/blackswan and follow the prompts. Complimentary Guest Pass
subject to Fitness First Terms and Conditions of Entry (Full Terms and Conditions and for a
list of participating Fitness First Clubs go to www.fitnessfirst.com.au/corporate/terms
11. Complimentary Guest Pass valid for first time visitors and local residents 16 years and
older only. The Guest Pass entitles the named entrant to a 3 Day Guest Membership at the
selected Fitness First clubs. Photo ID is required for entry. Strictly only 1 Guest Pass can
be used per person. Valid for 3 consecutive days commencing and including the date the
Guest Pass is redeemed. Not valid with any other offer. Valid only at club where Guest
Pass is redeemed. Guest Passes that have been photocopied, altered or damaged will not
be accepted. Guest Pass must be redeemed and used by 31/12/2010.
12. Total value of Major prizes = $40,499.00 AUD. All taxes (excluding GST) which may be
payable as a consequence of receiving a prize is the sole responsibility of the winner.
13. It is a requirement that the winner of a prize must be a resident of Australia, to be awarded
the prize.
14. The winners will be notified by telephone and in writing within five (5) working days of the
draw and all names will be published in The Australian on 29/12/10.
15. Prizes are not transferable and are not redeemable for cash. The Promoter’s decision is
final and binding - no correspondence will be entered into. The Promoter accepts no
responsibility for late, lost or misdirected entries or other communications. Entries will be
deemed void if forged, manipulated or tampered with in any way.
16. If the winner’s is under 18 years of age, the prize will be awarded to winner’s parent or legal
guardian on their behalf.
17. If necessary, the promoter may conduct an unclaimed prize draw at the same time and
place as the original draw on the 21/03/11 in order to distribute any unclaimed prizes. All
non-winning entries will be included in the unclaimed prize draw. In the event of any
winners in the unclaimed prize draw, the winners will be notified by phone and writing within
five (5) working days of the draw and all names published in The Australian on 29/03/11.
18. In the event that any prize becomes unavailable for reasons beyond the Promoter’s control,
the Promoter may substitute a prize of equal or greater value, subject to any written
directions from the various lottery authorities.

19. Prize values are correct at time of printing but no responsibility is accepted for any variation
in the value of any prizes. The yoghurt varieties in Woolworths quoted in section 3 may vary
by state due to supply or availability issues by region during the time of the promotion.
20. Woolworths Limited is the issuer of the WISH Gift Card but is not the promoter of the offer
and is therefore not responsible for the prizes, products or services under the offer or the
fulfilment of the offer terms. Woolworths limited is responsible for the fulfilment of the terms
and conditions of the WISH Gift Card. WISH Gift Cards are redeemable at participating
stores only. Participating stores may change from time to time. WISH Gift Card terms and

21. Fitness First is acting in partnership with The Trustee for C & C Sarris Family Trust T/A
Poseidon Dips Pty Ltd / Black Swan but is not the promoter of the offer and is therefore not
responsible for the prizes, products or services under the offer or the fulfilment of the offer
terms. Fitness First is responsible for the fulfilment of the terms and conditions of the
Platinum Gym Membership & Personal Training sessions. The Membership & Personal
Training sessions are redeemable at participating Fitness First clubs only. Participating

22. The Promoter assumes no responsibility for any failure to receive an entry or for inaccurate
information or for any loss, damage or injury as a result of technical or telecommunications
problems, including security breaches. If such problems arise, then the Promoter may
(where necessary with the approval of the relevant lottery authority) modify the promotion.
23. The Promoter is not liable for any loss or damage whatsoever which is suffered, including
but not limited to indirect or consequential loss, or for personal injury suffered or sustained
during the course of accepting or using the prize, except for any liability which cannot be
excluded by law.
24. Details from entries will be collected and used for the purposes of conducting this
promotion (which may include disclosure to third parties for the purpose of processing and
conducting the promotion) and for promotional purposes surrounding this promotion. By
entering this promotion entrants consent to the use of their information as described and
agree that the Promoter may use this information, or disclose it to other organisations that
may use it, in any media for future promotional purposes without any further reference or
payment to the entrant. Entrants may access, change and/or update their personal
information and obtain a copy of the Promoter’s privacy policy by contacting the Promoter
on (03) 9551 0611 during office hours Monday-Friday 9am-5pm.
25. To the extent permitted by law, the Promoter, its employees, and agents shall not be liable
for any claims, damages, injuries, costs, and expenses suffered, sustained or incurred
(including but not limited to indirect or consequential loss) as a result of, or arising out of, or
in any way connected with this competition and/or its prizes or any act or omission (whether
negligent or not) of the Promoter, its employees and agents.
