Entries for The Adjustment Bureau open on Thu 30 Jun 2011.

1.Information on how to enter and prizes forms part of the Conditions of Entry.

2.Entry is free and open to all residents of Australia, 18 years and over, with a current Civic Video membership (“Members”). Employees and their immediate families of the promoter, participating outlets, associated companies and its agencies associated with the promotion are ineligible to enter. The promoter is Civic Video Pty Ltd (67 003 851 152) 110 Bourke Rd Alexandria NSW 2015.

3.The competition commences 29th June 2011 and ends 1st August 2011 at 4pm. The winners will be drawn at Civic Video Pty Ltd 110 Bourke Rd, Alexandria NSW 2015 on 15th August 2011 at 4pm.

4.Limit one entry per rental or purchase transaction of “THE ADJUSTMENT BUREAU”.  Entry MUST be on original entry form provided by store or via official competition page on the Civic Video website (URL: http://www.civicvideo.com.au/competitions/enter.asp? ContentID=201107_theadjustmentbureau_promo). Proof of purchase must be presented to claim prize if winner declares purchase as validation for entry.

5.To enter customers must rent or buy “THE ADJUSTMENT BUREAU” during the promotion period (29th June 2011 and 1st August 2011) at Civic Video Pty Ltd., complete the entry form at the counter and place it in the draw or enter online as instructed above.

6.The first 3 valid entries from NSW and QLD (6 in total), the first 2 valid entries from WA (2 in total), the first valid entry from VIC (1 in total), the first valid entry from ACT, NT or SA combined (1 in total) will each win a 42" OHKI LED TV Full High Definition (RRP$999), Vivo Home Theater Speaker System with Subwoofer & DVD Player (RRP$149), HDMI Cable (RRP$49) & The Adjustment Bureau on DVD (RRP$29.95).

7.Total prize quantities & values for each state as follows:
NSW: 3 x $1226.95 = $3,680.85
QLD: 3 x $1226.95 = $3,680.85
WA: 2 x $1226.95 = $2,453.90
VIC: 1 x $1226.95 = $1,226.95
ACT, NT or SA: 1 x $1226.95 = $1,226.95

1.Total number of prizes nationally is 10 and total prize pool is valued at up to $12,269.50.

2.The promoter shall not be liable for any loss or damage suffered or sustained as a result of the prize.

3.Winner(s) will be notified by mail and The Australian newspaper on 29th August.

4.The prize, and any portion of the prize, is not transferable or redeemable for cash.

5.Unclaimed prizes will be drawn on 21st November 2011 at Civic Video Pty Ltd, 110 Bourke Rd Alexandria at 4pm. Winner(s) from unclaimed prize
drawn will be notified by mail and The Australian newspaper on 5th December 2011.

6.Normal membership conditions apply.

7.The judges’ decision in relation to any aspect of the competition is final and binding to any person who enters. No correspondence will be entered into.

8.The promoter may use the entrants’ names and addresses for future promotional, marketing and publicity purposes.

9.The Promoter reserves the right to request that the winner provide proof of age and identity prior to awarding the prize.  Identification considered suitable for verification will be at the discretion of the Promoter.
