Council is the largest provider of leisure and recreational services in Fairfield City. We’re conducting a survey to find out what you think of our recreational and leisure facilities to plan for the future of our City. We want to know what you think is good, what you would like to see more of and what can be improved. We’re doing this to ensure our City is a great place for your leisure and recreational activities. We encourage everyone to take part in the survey, whether you live in the City or simply utilise our facilities. Authorised under NSW Permit Number LTPS 11/05246 Click here to view Terms and Conditions How to make good lifestyle moves Recreation Needs Survey Terms and Conditions • The competition is open Australian residents who live within or utilise the recreation and leisure facilities in Fairfield City. • To enter the competition, participants must complete the Recreation Needs Survey online at www.fairfieldcity.nsw.gov.au or at any of the following locations: �� Council Administration Building (86 Avoca Road, Wakeley) �� Whitlam Library (Railway Pde, Cabramatta) �� Smithfield Library (Cnr Clancy and Oxford Sts, Smithfield) �� Wetherill Park Library (Stockland Town Centre, Polding St, Wetherill Park) �� Bonnyrigg Library (Bonnyrigg Plaza, Bonnyrigg Ave, Bonnyrigg) �� Fairfield Library (Cnr Barbara and Kenyon Sts, Fairfield) �� Cabravale Leisure Centre (Broomfield St, Cabramatta) �� Fairfield Leisure Centre (Vine St, Fairfield) �� Prairiewood Leisure Centre (Prairie Vale Rd, Prairiewood) Entrants are required to complete all the survey to enter. �� Completed entries can be handed in to any of the above locations or posted to ‘Fairfield City Council Recreation Needs Survey, Attention Brian Long, PO Box 21, Fairfield NSW 1860. �� The competition is open from 9.00am, Friday 10 June 2011 and entries close 4:30pm, Wednesday 31 August 2011. • Five prizes to be won. �� First prize is an IPAD valued at $600. �� Second prize is a $100 shopping voucher �� Third prize is a $100 shopping voucher �� Fourth prize is a $100 shopping voucher • The winners will be randomly drawn by a computer generator. Each survey will be assigned a sequential number. The draw will take place on Thursday 1 September 2011 at the offices of Quinntessential Marketing Consultant, Level 5, Suite 501, 5 Elizabeth Street, Sydney, NSW 2000. • Winners’ names will be published in the Fairfield City Champion on Wednesday 7 September 2011. • The decision of the drawer will be final and no correspondence will be entered into |
