Terms and conditions

01.
The name of the game is: 'Psoriasis Uncovered - Chance to win a $500 Visa Prepaid Card.'

02.
The game starts at 9 am on 28 October 2011 and ends at 5 pm 9 December 2011.

03.
Entry into the game is gained by patrons: by completing and submitting either online at www.psoriasissurvey.com.au or in hard copy where surveys are available a ‘Psoriasis Uncovered’ survey, including the ‘Chance to win a $500 Prepaid Visa Card’ section with the entrant’s name, address and daytime telephone number.

04.
Entry into the game is restricted to Australian residents only.

05.
Entry is limited to one entry per person.

06.
Employees of Abbott Australasia Pty Ltd (“Abbott”) and their immediate family, its associated companies and agencies are ineligible to enter.

07.
Subject to satisfying the entry requirements, a patron will gain entry into the game.

08.
Entry must be received before the time the draw is conducted.

09.
The draw will be conducted on 16 December 2011 at 10 am.

10.
The place where the draw will be undertaken is: Stollznow Research, Level 2, 256 Military Road, Neutral Bay, NSW, 2089.

11.
The winning entrant wins a $500 Visa Prepaid Card.

12.
The winner’s details will not be published.

13.
The use of the Visa Prepaid Card is subject to the terms and conditions set out by Visa. The prize is not transferable or exchangeable and cannot be taken as cash.

14.
Should the winner not be able to be contacted within 3 months, an unclaimed prize draw will be held at the same time and venue as in clause (10) on 16 March 2012.

15.
The winning entrant will be notified by mail. Only the winner’s contact details will be passed on to Abbott. All other entrants’ details will be kept confidential in compliance with the Privacy Act.

16.
The winning entrant will be notified of the method for claiming the Visa Prepaid card when he/she is notified that he/she has won.

17.
Abbott reserves the right to request the winning entrant to provide proof of identity, and/or proof of entry validity. Proof of identification, and entry considered suitable for verification is at the discretion of Abbott. In the event that the winning entrant cannot provide suitable proof, the winning entrant will be forfeited their prize in whole and no substitute will be offered.

18.
If for any reason this game is interfered with or is not capable of being conducted as planned, whether it be caused by computer virus, telephone failure, tampering, unauthorised intervention, fraud, technical failures or any other cause beyond the control of Abbott which corrupt or affect the administration, security, fairness or integrity of the game, Abbott reserves the right as its sole discretion, (subject to state regulations), to cancel, terminate, modify or suspend the game as appropriate.

19.
Abbott may at its sole discretion disqualify any individual who tampers with the entry process. Except for any liability that cannot be excluded by law, Abbott (including its officers, employees and agents) excludes all liability (including negligence) for any loss or damage (including loss of opportunity); whether direct, indirect, special or consequential or arising in any way out this game, including, but not limited to, where arising out of the following: (a) any technical difficulties or equipment malfunction (whether or not under Abbott’s control); (b) any theft, unauthorised access or third party interference; (c) any entry or prize claim that is late, lost, altered, damaged or misdirected (whether or not after their receipt by Abbott); due to any reason beyond the reasonable control of Abbott, (d) any variation in prize value to that stated in these Terms and Conditions; or (e) any tax liability incurred by a winner or entrant.

20.
Any entries not complying with these terms and conditions are invalid.
