Sample Offer Terms and conditions:
1. This Pronamel Gentle Whitening complimentary sample offer is open to residents of Australia only.
2. Limit one sample per person.
3. To be eligible to receive one complimentary 25g sample of Pronamel Gentle Whitening product (RRP
AU$2.25), a claimant must submit a claim form so that the Promoter receives it between 1 February 2013
and 30 April 2013.
4. The sample will be sent after 30 April 2013. The sample will be sent by standard post to the Australian
address submitted on the claim form. Postage tracking will not be provided for the sample.
5. A total of 10,000 samples are available to be distributed as part of this sample offer. In the event that before
30 April 2013 the sample supplies are exhausted, the sample offer will finish at that time.
6. The information shown is provided by GlaxoSmithKline Australia Pty Ltd (“GSK”) and the Promoter makes
no representation in relation to the product.
7. Employees of the Promoter, suppliers and their agencies (including resellers and other agencies associated
with this offer) and their immediate families are ineligible to participate in this sample offer.
8. Eligibility to claim the product sample is conditional upon provision of the claimant’s contact information.
9. Claimants’ personal information ("PI") will be collected to enable the Promoter to administer and promote this
offer and, if a claimant consents, may be used to market its services to the claimant. The PI of claimants
may be provided to others assisting, including suppliers and deliverers. Claimants may contact the Promoter
at the address given below to request access to any of their collected PI that is still held. If a claimant does
not truthfully provide all requested PI, the Promoter may determine that they are not eligible to claim a
sample. If a claimant wishes to receive information about news, events and offers (including via electronic
means) from GSK, they may tick the opt-in box at the time of claiming. Without limiting the Promoter’s
privacy policy, by claiming this offer and indicating their relevant consent, a claimant also agrees to the
Promoter sharing their personal information with GSK for the purposes of conducting this sample offer. By

10. The Promoter reserves the right to verify the information provided and reject incomplete or inaccurate claim
