Win one of three prizes when you complete
your SiSU health check
We’re proud to expand our healthcare services to help support the wellbeing of the
Lithgow community. The SiSU health station enables individuals to partake in a free,
self-service health check in less than five minutes. To celebrate, we’re giving Lithgow
residents (including residents in surrounding areas) a chance to win one of three
prizes by completing their SiSU health check at the Lithgow Care Centre. To enter,
you’ll need to scan the QR code after you have completed your health check and fill
out the online form on our website. When you go into the draw, you have a chance to
win a Withings Body + Body Composition Wi-Fi Scale (Black) valued at $179, a
Marley Spoon gift card valued at $100 or a Tin Shed gift voucher valued at $100.
The competition will run from 1 June – 31 August 2023

Terms & Conditions
Information on how to enter and prizes form part of these Terms and Conditions and entry into
this promotion is deemed to be acceptance of these Terms and Conditions.
1. Entry is only open to Lithgow residents and residents in surrounding areas who complete their
SiSU health check at the Westfund Lithgow Care Centre - 120 Main Street Lithgow NSW 2790
Australia. Westfund employees and their immediate families are ineligible to enter or win.
2. THE PRIZES: Between June and August 2023, we’ll be giving away three prizes.
a) Prize one for the month of June: Withings Body + Body Composition Wi-Fi Scale (Black)
valued at $179.
b) Prize two for the month of July: Marley Spoon gift card valued at $100.
c) Prize three for the month of August: The Tin Shed gift voucher valued at $100.
Total prize pool of $379 (plus the cost of postage).
3. HOW TO ENTER: Entry is by scanning the QR code at the Lithgow SiSU health station after the
health check is complete. Entrants must complete the online form in order to go in the draw to
win. Entrants are allowed one entry per prize draw.
4. Competition commences at 9 am Thursday 1 June 2023 and closes at 4:30 pm Thursday 31
August 2023. The draws will take place at Westfund Head Office, Lithgow. The winners will be
officially notified by phone call.
a) Entries for prize one must be completed between 9 am Thursday 1 June 2023 and
3:30 pm Friday 30 June 2023. The draw for prize one will take place at 4:30 pm on
Friday 30 June 2023.
b) Entries for prize two must be completed between 9 am Saturday 1 July 2023 and
3:30 pm Monday 31 July 2023. The draw for prize two will take place at 4:30 pm on
Monday 31 July 2023.
c) Entries for prize three must be completed between 9 am Tuesday 1 August 2023 and
3:30 pm Thursday 31 August 2023. The draw for prize three will take place at 4:30
pm on Thursday 31 August 2023.
5. This is a random prize draw; no alternative prize will be offered.
6. Entrants must be over 18 years of age.
7. Westfund reserves the right to verify the membership details and validity of entrants. Westfund
reserves the right to disqualify any entrant for interfering with the entry process or for submitting
an entry which is not in accordance with these Conditions of Entry.
8. Our announcement of the competition winners is final and no correspondence will be entered
into.
9. The prizes are not transferable or exchangeable and cannot be taken as cash. We cannot accept
any responsibility for any variation in the prize or the prize value.
10. Gift cards are subject to the terms and conditions of the supplier.
11. The winning entrants agree to have their name and/or image published on Facebook and
through Westfund’s website: www.westfund.com.au/competition-winners.
12. Westfund is not liable for any loss or damage suffered (including but not limited to indirect or
consequential loss) or for any personal injury suffered or sustained as a consequence of the use
and enjoyment of your prize.
13. No responsibility is accepted for late entries after the promotion period has lapsed.
14. All entries become and remain the property of Westfund. We collect personal information about
you to enable you to participate in this promotion. We may need to disclose your personal
information to relevant State and Territory lottery departments. Winners' names will be published
as required under the relevant lottery legislation. If you do not provide your personal information,
Westfund will not be able to enter you into this promotion. In most cases, you can gain access to

15. Promotional materials are void if stolen, forged, mutilated or tampered with in any way.
16. Printing errors and other quality control matters will not invalidate an otherwise valid prize claim.
All valid claims for prizes and entitlements will be met.
17. UNCLAIMED PRIZE DRAW: If the prizes are unclaimed by midnight Monday 11 September
2023 any unclaimed prize draw will take place at 12 noon on Tuesday 12 September 2023 at
Westfund Head Office Lithgow, NSW. The winner will be notified via phone.
18. Prizes will be sent via Australia Post. The promoter cannot be held responsible for damaged,
undelivered, lost or stolen item and no replacement will be provided.
19. This competition is not associated in any way with Withings, Marley Spoon or The Tin Shed.
