Worked fine for me. :)  I got a 'Thankyou" message, after my entry went through. :)

loaded up okay for me but got this message when I submitted:

Error loading Partial View script (file: ~/Views/MacroPartials/InsertUmbracoForm.cshtml)

There was no field for email address which might be the problem as they want to enter you onto their data base for offers which I would assume means an email address.  So not sure if I'm entered or not!  Will have another go later.
