1. Taking part in this incentive constitutes acceptance of these terms and conditions.
2. Incentive commences:
▪ at 9am on the 1st
January 2022 and closes on 12pm 30th
June 2022.
3. The final winner will be drawn on Monday 04th
July 2022. The first correctly completed entry
will be considered the winner.
4. Winners will be notified by phone and will need to receive their gift card in person at their
local NWRH location. Photo identification is required.
5. The promoter of the incentive is North and West Remote Health (NWRH) Ltd, (ABN 881 505

6. Entry is open to all clients and/or carers (past and current) of North and West Remote
Health; one entry per person.
7. Paid and volunteer employees (past and current) of the promoter (and their immediate
families) are not eligible to enter.
8. To enter the incentive and be in the running to win a $500 Gift Voucher, entrants must:
▪ Complete the survey (17 Questions; approx. 5 minutes)
▪ Complete their details (including name, address, postcode and phone) at the end of
the survey

Mastercard® prepaid cards are accepted for electronic transactions (excluding transactions
