To receive your $25 cash back simply upload a scan of your proof of purchase and activation and submit it via this form no later than 20th January 2015. Your cash back will be deposited into your bank account within 60 days of receiving the completed claim form. For multiple purchases on one receipt, each product must be claimed individually. Offer valid between 7th November 2014 and 31st December 2014.
