TERMS AND CONDITIONS
1. The promoter is the BreastScreen SA (hereafter referred to as “BSSA”), 167 Flinders
Street, ADELAIDE, SA 5000 (ABN 96 269 526 412)
South Australian Department for Health and Wellbeing (hereafter referred to as “SA
Health”), PO Box 287 Rundle Mall, ADELAIDE SA 5000 (ABN 97 643 356 590).
ENTRY
2. Entry is open to South Australian residents only who have a screening
mammogram with BreastScreen SA during the months of January and February
2021. Only one entry per person is permitted.
3. The “promotional period” will run as follows:

4. Participants must call and make a booking with BSSA and undergo a breast screen to
be able to enter the competition and be eligible to claim the prize.
5. To enter, participants must:
5.1 have a breast screen during the promotional period; 1 to 30 January 2021 or 1
to 27 February 2021.
5.2 attend their breast screen appointment to be eligible for the competition and to
claim their prize
5.3 be women aged over 40 years of age and meet the eligible screening criteria
outlined in the BreastScreen SA Information and Consent for Screening form.
6. A participant is only eligible to claim their winning prize once the participant has
attended their breast screen appointment.
7. By entering into and participating in this promotion, each entrant fully and
unconditionally agrees to and accepts these terms and conditions.
8. By registering in the competition, each entrant agrees for BSSA to contact them by
email and/or phone to notify them if they win a prize.
9. By registering in the competition, each entrant agrees for BSSA to contact them via
SMS and/or email during the promotional period to remind them of the competition.
10. BSSA reserves the right to request winners to provide proof of identity, proof of South
Australian residency status and/or proof of entry validity, in order to claim a prize.
Acceptance of proof of identification, residency and entry considered suitable for
verification is at the discretion of BSSA. In the event that a winner cannot provide
proof, the winner will forfeit the prize in its entirety and no substitute will be offered.
11. All promotional material including information on advertisements, websites, and list of
terms and conditions form part of the conditions of entry.

13. The total value of all prizes is $3,000. The prize values listed are in Australian dollars.
14. Each prize remains the property of BSSA until claimed by the prize winner.
15. All prizes will be drawn at the offices of BSSA at 167 Flinders Street, Adelaide. There
will be two prize drawers
• Five winners will be drawn 4pm, Thursday 11 February 2021
• Five winners will be drawn 4pm, Thursday 11 March 2021
16. The winners of all prizes will be determined by the first valid entry randomly selected by
a computerised system at BSSA, managed by a BSSA employee.
17. Winners will be notified by mail, email or telephone. However, the winner is only
eligible to claim their prize once they have attended their breast screen appointment.
All reasonable steps to notify winners will be taken by SA Health. The winners will be
required to provide valid proof of identification to claim their prize.
18. BSSA accepts no responsibility or liability if a winner is unable to be contacted, and all
methods of communication are unsuccessful.
19. All prizes are non-transferable and cannot be redeemed for cash.
20. BSSA reserves the right to verify the validity of all entries and disqualify any participant
for tampering with the eligibility process not in accordance with these terms and
conditions.
21. BSSA accepts no responsibility for any tax implications that may arise from the prize
winnings.
22. Should any of the prizes not be claimed within 90 days of the draw they will be
considered forfeited.
FINAL CONDITIONS
23. Participation in the promotion means acceptance of these terms and conditions as
listed. Failure to observe these Terms and Conditions may result in disqualification.
24. By entering the competition, the participant agrees to participate in any reasonable
publicity or advertising that BSSA may request, including photographs and use of their
image for promotional purposes as requested by BSSA.
25. BSSA will not be liable for any loss (including, without limitation, indirect or
consequential loss), damage, personal injury or death which is suffered or sustained
(including but not limited to that arising from any person's negligence) in connection
with in entering the competition or taking or using the prizes, except any liability that
cannot be excluded by law (in which case that liability is limited to the minimum
allowable by law).
26. BSSA’s decision is final and no correspondence will be entered into.
27. BSSA assumes no responsibility for any error, omission, interruption, deletion, defect,
delay in operation or transmission, communications line failure, theft or destruction or
unauthorised access to, or alterations of entries, and reserves the right to take any
action that may be available.
28. If for any reason this competition is not capable of running as planned due to causes
including, but not limited to tampering, unauthorised intervention, fraud, technical
failures or any other causes beyond the control of BSSA which corrupt or affect the
administration security, fairness, integrity or proper conduct of this competition, BSSA
reserves the right in its sole discretion, to the fullest extent permitted by law, to
disqualify any individual who tampers with the entry process, take any action that may
be available, and to cancel, terminate, modify or suspend the competition (subject to
State regulations, if any).
29. All details of the entrants will be used and preserved in accordance with the National
Information Privacy Principles as per the SA Government Privacy Information
