National Pharmacies is offering SA, VIC and NSW members, the opportunity to WIN a Disney on Ice presents Worlds of Fantasy prize pack, including four tickets for the family to see the show in their capital city!

Prizes includes four tickets to a selected performance of Disney on Ice presents Worlds of Fantasy in the capital city of the winner's residential state (SA, VIC and NSW only); Disney on Ice Pennant, photo frame, colouring book, stuffed toy - young simba from the Lion King, Purse toy - Minnie Mouse, Show program, Floss hat-flower, Floss hat-mater foam (From Cars), Snow mug-princess, Cutout fairies, Tinkerbell spinner wand.

There is one prize pack for SA, VIC and NSW and each prize is valued at $342.50. Total prize pool is $1,027.50.

Entrants must be a financial member of National Pharmacies, fill out an entry form available online at www.nationalpharmacies.com.au, and tell us their favourite Disney movie memory.

The competition will start on Monday 2 May and will go through until Sunday 5 June 2011.


Terms and conditions:

1.  Entry into this draw is deemed acceptance of and agreement to these rules.

2.  Information on how to enter and the prize form part of the conditions of entry.

3.  Entry into this draw is open to National Pharmacies members living in South Australia, Victoria and New South Wales, excluding employees and their immediate families of Friendly Society Medical Association Limited and their associated agencies and companies.

4.  Only financial National Pharmacies members can enter this competition. To enter, members must fill out an entry form online at www.nationalpharmacies.com.au, answer the question: “What is your favourite Disney movie memory?” by Midnight Sunday 5 June 2011.

5.  Entrants will be put into the draw for their residential state, as indicated in their entry. One winner will be drawn for each state (South Australia, Victoria and New South Wales). The winners will receive four (4) tickets to a selected performance of Disney On Ice presents Worlds of Fantasy in the capital city of the their residential state (SA, Vic and NSW only); official show programme; Disney on Ice Pennant, photo frame, colouring book, stuffed toy - young simba from the Lion King, Purse toy - Minnie Mouse, Floss hat-flower, Floss hat-mater foam (From Cars), Snow mug-princess, Cutout fairies, Tinkerbell spinner wand. There is one prize pack for SA, Vic and NSW and each prize is valued at $342.50. Total prize pool is $1,027.50.

6.   The tickets will be for one of the following dates: Adelaide - 21- 24 July, Melbourne - 6 - 11 July, Sydney - 13 -18 July. Total value of each prize is $342.50.

7.  Any additional services and costs involved must be covered by the prize winner, including but not limited to transportation to and from the venue.

8.  Winners must be available to attend the performance on the date designated above.

9.  The winners will be selected based on their answer to the question:
“What is you favourite Disney movie memory?”, and selected 12pm Friday 10 June 2011 at the promoter’s premises: Level 5, 52 Gawler Place, Adelaide SA 5000.

10.The winner will be notified by telephone and post on Friday 10 June 2011. A public announcement will also be posted online on the National Pharmacies website: www.nationalpharmacies.com.au/

11.Prizes are not transferable and are not redeemable for cash.

12.The promoter is Friendly Society Medical Association Limited ABN 69 088 347 602 of Level 1, 52 Gawler Place, Adelaide South Australia 5000.

13.All entries remain the property of the promoter.

14.Any change in the value of the prize occurring between publishing date and the date the prize is claimed is not the responsibility of the promoter.

15.If the prize is not claimed within 5 days of the draw date, an unclaimed prize draw will be conducted on 15th June 2011 subject to any written directions given under Reg 37 of the Lottery and Gaming Regulations 1993 (SA).

16.Winners of the unclaimed prize draw will be notified by telephone and post and on 15th June 2011.

17.The winners, and the persons they nominate to take, release the promoter and other organisers, their related bodies corporate, affiliates, officers, agents and employees, from all liabilities, loss and damage of any kind arising at any time out of or in connection with the acceptance of, and participation in, the prize.

18.One entry per member.
