TERMS AND CONDITIONS

METHOD OF ENTRY

1.  Information on how to enter and specifications of the prize form part of these Terms and Conditions.

2. Entry is open to Australian residents aged 18 and over only. By entering this competition, entrants will be deemed to agree with these terms of entry.

3. The MAGGIE G ‘Win a Teeth Whitening Package’ competition commences on Tuesday, May 24th 2011 at 08:00 AEDT and ends on Friday, June 24th 2011 at midday 12:00 AEDT (the Promotion Period).

4. Entrants must register as a subscriber to the MAGGIE G website at either www.MaggieG.com.au or www.facebook.com/Maggie.G.Exclusive and enter all of the details asked for to complete the online entry form. Entrants will also upload their entry to the following competition via this form: Tell us in 25 words or less why a confident smile is important to you.

5. Entrants must include their first name, email address, state and postcode of residence.

6. Only one entry per email address may be submitted. All entries become and remain the property of MAGGIE G.

GRAND PRIZE

The Grand Prize winner will receive the following:

1.A medical grade teeth whitening package administered by a professional dentist, recognized by the Dental Board of NSW of THE TOOTH PLACE. Located at Suite 108, 24-30 Springfield Avenue, POTTS POINT NSW 2010.
2.$50 worth of credit used towards any other dental service provided by THE TOOTH PLACE. (Grand Prize total is valued at $549)
 THE TOOTH PLACE TERMS AND CONDITIONS

•Teeth Whitening Consultation must take place within 3 months of notification.
•Treatment not suitable for persons under 18 years of age.
•Appointment is essential, please call 02 9331 2273.
•Appointments are available Wednesdays, Thursdays and Fridays only.
•Upon professional consultation, if teeth or gums are deemed unsuitable to teeth whitening, the consulting doctor reserves the right to not proceed with treatment until the dental needs are met.
•Upon professional consultation, if the consulting doctor deems the prize winner unsuitable to treatment for personal reasons, they reserve the right to not proceed with treatment and MAGGIE G will award the prizewinner MAGGIE G credits to the value of $549. This credit may be used toward purchasing any other MAGGIE G exclusive offers.
•Results may vary depending upon the initial shade of the teeth.
7. All valid entries are automatically entered into the Grand Prize draw. The Grand Prize winner will be selected by MAGGIE G on Monday, June 27th, 2011 at 15:00 AEDT. A MAGGIE G Marketing representative will contact the Grand Prize winner by notification via email, where a contact telephone number will be requested.
If the Grand Prize winner cannot be contacted by Monday, July 11th 2011 at 17:00 AEST, MAGGIE G will select a new Grand Prize winner and will follow the same method of contact as with the original selected winner.

8. MAGGIE accepts no responsibility for any loss, damages, discontent with treatment, product or service or injury incurred in connection with taking up the prize at THE TOOTH PLACE.

9. The Grand Prize is not transferable or exchangeable and cannot be taken as cash.

WINNER NOTIFICATION

10. The Grand Prizewinner will be notified by phone and/or email/mail. The name of the Grand Prize winner will be published on the MAGGIE G facebook page www.facebook.com/Maggie.G.Exclusive on Tuesday, June 28th 2011.

FINAL CONDITIONS

11. This is a game of skill. Chance plays no part in determining the winner. Each entry will be individually judged, based upon literary and creative merit. All entries must be the original independent creation of the entrant and free of any claims that they infringe any third party rights. Entries must not have been published previously and/or have been used to win prizes in any other competitions.

12. The judges reserve the right to disqualify any entrant submitting an entry, which in the opinion of the judges includes objectionable content, including but not limited to profanity, nudity, potentially insulting, scandalous, inflammatory or defamatory images or language. The judges' decision will be final and no correspondence will be entered into.

13. All entries become and remain the property of MAGGIE G. MAGGIE G may use the entrants' names and addresses for promotional, marketing and publicity purposes.

14. MAGGIE G and THE TOOTH PLACE assume no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alterations of entries, and reserves the right to take any action that may be available. If for any reason this competition is not capable of running as planned, due to causes including, but not limited to tampering, unauthorized intervention, fraud, technical failures or any other causes beyond the control of the promoter which corrupt or affect the administration security, fairness, integrity or proper conduct of this competition, the promoter reserves the rights in its sole discretion to disqualify any individual who tampers with the entry process, take any action that may be available, and to cancel, terminate, modify or suspend the competition, Subject to State Regulation.

18. All details of the entrants will be used and preserved in accordance with the National Privacy Principles. A request to access, update or correct any information should be directed to that office. Unless otherwise notified, MAGGIE G may use the entries and details of entrants for promotional and publicity purposes. Entrants consent to their full names and/or their entries being used in all media by MAGGIE G.

19. MAGGIE G will not be held liable for any loss, damage or injury of the results directly or indirectly from the Winner taking part in any aspect of the Prize.
