Complete a 2 minute online survey about your life insurance company to enter.

You need to have owned (or been the beneficiary of) one of the following policies in the last 12 months:

Life Insurance
TPD Insurance
Trauma Insurance
Income Protection Insurance, or
Funeral Insurance
You will be asked to rate the insurer on communications, application process and service and provide a comment on your overall experience and whether you would recommend your insurer to others.

Insurance held through your super fund is acceptable (the insurer's name will be listed on your super fund's website).

Genuine reviews only should be submitted as the winner will need to provide evidence of policy ownership and a photo ID to claim the prize.
