Terms and conditions

Information on how to enter and prizes forms part of these terms and conditions of entry. The promoter is Australian Unity Health Care Limited ABN 80 10 9 450 991, 271 Spring St, Melbourne VIC 3000 (Promoter). 

Prize

    The prize consists of one $1,000 Visa Gift Card (prize total value is $1,000).
    Gift card is issued by Cuscal Ltd ABN 95 087 822 455 AFSL 244116. Cuscal Ltd deals with any funds loaded onto the card in accordance with its arrangements with Corporate Prepaid Cards Pty Ltd ABN 25 092 828 772 AFSL 509642.
    Gift card will be delivered via registered post. Australian Unity will not be held responsible for any damaged, undelivered, lost or stolen gift card and no replacement will be provided. Visa Gift Cards are subject to the terms and conditions that appear on the gift card letter or at www.universalgiftcard.com.au

    Entry
    To enter, the eligible entrant must book, and attend, a dental appointment at any of our five Australian Unity Dental Clinics. Bookings can be made in clinic, via the Australian Unity Dental contact centre or online at www.australianunity.com.au/dental
    Entry is limited to one entry per person. There is no fee to enter.
    The competition period runs from 12.01am 13/02/2023 to 8:00pm (AEST) 15/06/2023.

    Eligibility
    To be eligible for the prize draw a person must be:
        18 years or older; and
        attend a dental appointment at an Australian Unity Dental Clinic during the competition period.
    Employees of the Promoter or any of its related bodies corporate, and the immediate family members of those employees, are ineligible to enter this competition.

    Prize draw and notification
    The random prize draw will be held on 19/06/2023 at 10:30am AEST at 271 Spring Street, Melbourne, VIC, 3000 and the winner notified within two days of the draw by phone and email using the telephone number and email address provided. The winner will also be published on the Australian Unity website at australianunity.com.au/competitionwinners on 21/06/2023.     
    If the prize is not claimed by 10.00am on 26/06/2023 a second chance random draw will take place on this date and the prize-winner will be notified by phone and email, and the new winner will be published on the Australian Unity website at australianunity.com.au/competitionwinners on 28/06/2023.
    The prize is not transferable or refundable.  If any aspect of a prize cannot be delivered for any reason, the Promoter reserves the right to substitute the prize for any other item (or items) of equal or greater value subject to State laws.
    The Promoter may require the winner to provide proof of eligibility, identity, proof of age and proof of residency at the nominated prize delivery address.  Identification considered suitable for verification is at the Promoter’s discretion.

    Other conditions
    All entries will be the property of the Promoter. The information entrants provide will be used by the Promoter for the purpose of conducting this competition and communicating with entrants as to future offers, information and/or marketing promotions. The Promoter may disclose entrants’ personal information to its contractors and agents to assist in conducting this Promotion or communicating with entrants. The Promoter is bound by the Australian Privacy Principles in the Privacy Act 1988. The Promoter’s privacy policy is available on the Promoter’s website at australianunity.com.au/privacy-policy. Entrants can request access to the personal information the Promoter holds about them by contacting the Promoter at the address set out above.
    The winner of the promotion consents to the Promoter using their name, likeness, image and/or voice (including photograph, film and/or recording of the same) in any media, marketing or advertising activities for an unlimited period without remuneration.
    If for any reason beyond the reasonable control of the Promoter, this promotion is not capable of being conducted as reasonably anticipated, the Promoter reserves the right, in its sole discretion, unless to do so would be prohibited by law, to (a) disqualify any entrant; and/or (b) subject to any written directions from a regulatory authority, to cancel, suspend, modify, terminate or cancel this promotion.
    The Promoter’s decisions in relation to all aspects of this promotion are final and binding on all entrants.
    To the extent permitted by law, the Promoter shall not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) which is suffered or sustained by any entrant/winner as a result of participating in this promotion or accepting any prize.
    The Promoter accepts no responsibility for any tax implications that may arise from winning the prize. Independent financial advice should be sought.
    The laws of Victoria apply to this promotion to the exclusion of any other law.  Entrants submit to the non-exclusive jurisdiction of the courts of Victoria.
