CONDITIONS OF ENTRY
1. This information on how to enter the competition form part of the conditions of
entry. Entry into the competition is deemed acceptance of these conditions.

2. The promoter is Terry White Chemists Charlestown. ABN 76119881770 and Terry
White Chemists Hamilton ABN 22642052904.

3. Subject to the condition in clause 4, all Tony Ferguson registered customers who
purchase a fortnights supply of TF diet shakes(ie: 28 shakes) between Jan 10 and
May 6 are entitled to enter into the draw to win a York Treadmill valued at
$1099.99. Each and every purchase of 28 shakes is entitled to one entry during
the said period.

4. To enter, eligible customers must complete the entry form provided in store and
place it in the entry box at Terry White Chemists Charlestown or Terry White
Chemists Charlestown.

5. To be valid, all details on the entry form must be complete and the entry form must
be an original. Incomplete and unintelligible entries will not be eligible. Copies will
not be accepted.

6. Entry is only open to Australian residents. (Just thought I'd add that the 2 stores holding the promotion are located in NSW at Charlestown and Hamilton - tigers)Employees (and their immediate
families) of the promoter, agencies and retail sponsors associated with this
promotion are ineligible to enter.

7. The competition commences on Monday 10 Jan 2011 and closed at 4pm on
Friday 6 May 2011. Entries received after this time will not be included in the
competition.

8. The prize is one York Performance treadmill valued at $1099.99.
9. The prize draw will take place at Terry White Chemists Charlestown at 4pm on
Friday May 6 2011. The first valid entry drawn on each of these days will win the
prize.
10. The winners will be notified by telephone the day after each prize draw. The
promoter will continue to attempt to the winners until 5.00pm on the day which is 7
days after the draw. If the promoter is unable to contact them, a re-draw will be
conducted at the same place as the original draw on the day that is 8 days after
the original draw.
11. The draw will be final and no correspondence will be entered into.
12. The prize is not transferable or exchangeable and cannot be exchanged for cash.
13. All entries become the property of the promoter. The promoter collects the
personal information on the entry form provided by entrants in the competition for
the purpose of the competition. By entering the competition, each entrant
acknowledges a further primary purpose of collection of entrants personal
information is for direct marketing, to contact entrants with marketing information
and to market improve and add to products and services that may be of interest to
entrants. The promoter may share information with third parties who provide
services to the promoter. By entering the competition and opting in at the point of
entry into the competition, each entrant agrees that the promoter may use the
entrant’s personal information in this way.
