1. Provide a brief description of the sport in which the Nominee is involved and why support is needed or justified. *

2. Provide a brief description of the athlete and their involvement in their chosen sport including training regime, background, resources (current and those required) and future goals. *

3. Provide a brief description of the way in which the student’s school is able or would like to be able, to support the student athlete in their sporting endeavours. *

4. Why should he/she/they be the Schultz Toomey O’Brien Lawyer Student Athlete of the Month?

Rules and Eligibility Criteria



Entries will only be accepted from students currently enrolled in schools located in the Sunshine Coast.



The awards are open to all Primary and Secondary Schools, Public and Private, on the Sunshine Coast.



Prizes will be awarded over a period of nine months. February, March, April, May, June, July, August, September and October. The major prize winner and “People’s Choice Award” will be awarded in November.



Each of the four main questions that require answering on the entry form will be given equal weighting by the judges when determining the monthly winner.



To be eligible for the major award or the “People’s Choice Award” you must have been a monthly winner on one of the previous nine months and remain enrolled at a school on the Sunshine Coast.



“People’s Choice Award” – all of the monthly winners will be highlighted in the Sunshine Coast Daily throughout November and the public will be encouraged to vote online or through entry forms available in the Sunshine Coast Daily. The person with the most votes will win the “People’s Choice Award”.



To be considered for a monthly award your entry must be received by the Sunshine Coast Daily no later than the 20th of that month.



Any entries received after this date will be held over until the next month.



The $5,000 advertising package that will be won by the school attended by the overall winner cannot be exchanged for

cash nor can it be transferred to another party. If a student changes schools after winning a monthly award, it is the

school that they are enrolled in at the time of winning the monthly award who will be the winner of this award.



The Sunshine Coast Daily Reader’s Award is based on luck only. It is open to anyone who submits a vote

on the “People’s Choice” entry form found in the Sunshine Coast Daily throughout November or by submitting their vote via the Sunshine Coast Daily website using the form available there. All of the readers who have submitted a vote will have their names placed in a barrel and four names will be randomly selected. Each of the four names pulled out will receive $500 from the Sunshine Coast Daily.



All entries must be submitted online through the Sunshine Coast Daily’s website link or on the approved Nomination form. This form can be obtained from your local school or downloaded from the Sunshine Coast Daily’s, Schultz Toomey O’Brien Lawyers or Hot 91.1’s website. You can also pick one up from the Sunshine Coast Daily, Schultz Toomey O’Brien Lawyers

of Hot 91.1’s offices.



The nominated judges may be replaced with another judge without the organisers giving notice or a reason as to the change.



The judges’ decision is fi nal and no correspondence will be entered into.



The breakfast supplied by Hot 91.1 will cater for a maximum of 300 children.



To host an Outside Broadcast Hot 91.1 will require access to 240v power and a stand-alone phone line.



The organisers reserve the right to change the day of the Outside Broadcast if the last Friday of the month falls

during school holidays, or for any other reason.



All entries and any information contained therein remain the property of the organisers. Any photos taken of the winners

remain the property of the organisers and may be used in future promotion of this event.



Nominees give their permission to have their photograph appear in the Sunshine Coast Daily.
