Thank you for using the Health and Wellbeing Directory to either refer a customer (must be a registered Health Practitioner) or for visiting (consumer) one of the City of Canada Bay's local Health and Wellbeing businesses listed in the 2011/2012 Health and Wellbeing Directory!
Enter this competition by filling in your details below and you will be in the running to WIN a month membership pass at Five Dock Leisure Centre. There is a free pass drawn every month until June 2012.
Prize includes:
A free month membership at Five Dock Leisure Centre, including a free fitness assessment and exercise program, valued at $137.
The more you refer or visit the participating local health and wellbeing businesses, the more chances you have at winning this fantastic prize!
Competition closes 30 June 2012, 4pm.
This promotion is permitted under TPL LTPM/11/00445.
1. Competition is open to customers of the participating businesses that have advertised in the Health and Wellbeing Directory 2011/2012.
2. Entry is open to residents of Australia. Employees of City of Canada Bay and their immediate families are ineligible to enter.
3. All entries are required to provide their name, contact details, receipt details for eligibility and for contact if they are a winner.
4. Council will not accept responsibility for any entries that were sent but not received.
5. The prize pool is valued at $1644.00 ($137 per month x 12 months)
6. The prize will include one month free membership at Five Dock Leisure Centre (Cnr Queens Road & William Street, Five Dock - 9744 2622 -
www.fdlc.com.au), including a free assessment and exercise program.
7. The prize is not transferable & cannot be taken as cash, unless otherwise stipulated.
8. Prize values are correct at time of printing but no responsibility is accepted for any variation in the value of any prizes.
9. The Promoter is not liable for any loss or damage whatsoever which is suffered, including but not limited to indirect or consequential loss, or for personal injury suffered or sustained during the course of accepting or using the prize, except for any liability which cannot be excluded by law.
