Entry into the new iPad competition is open to all patients attending participating Pacific Smiles Dental Centres and is subject to these terms and conditions. The promoter is Pacific Smiles Group Limited ABN 42 103 087 449. Information on how to enter forms is part of these terms and conditions. Entry into the competition is deemed acceptance of these terms and conditions. Employees or SFA Practitioners (or their immediate family members) of the promoter are not eligible to enter. The competition begins on 1st January 2015 and closes on 30th June 2015. The prize draw will take place at 10am on the 6th July 2015. The prize draw will be conducted at Unit 2, 595 Main Road Glendale NSW 2285. The prize winner will be notified by telephone and in writing. The prize must be claimed within 3 months of the prize draw taking place. If the prize winner cannot be contacted or does not claim the prize within the required timeframe, then the prize will be forfeited and a substitute prize/s will not be offered in lieu of the prize and a redraw will occur. The redraw will take place at 10am on the 6th October 2015 at Unit 2, 595 Main Road Glendale NSW 2285. The winner of the redraw (if applicable) will be notified by telephone and in writing. The prize will be mailed to the winner by registered post. The prize is not transferable or redeemable for cash. The prize winner will be announced at

To enter the competition and be eligible to win, patients must complete the online registration form located at the specified URL (as indicated on the mailing piece received by the patient) and request to receive future appointment reminders via email. All entries must be complete and successfully received by Pacific Smiles Group before the competition closes. Only one entry per patient. The prize consists of at 64GB new Apple iPad with Wi-Fi valued at $739.00 including GST. The prize does not include data or a contact with any telecommunications provider. No responsibility will be accepted by the Promoter for late, lost or misdirected entries and all entries are deemed to be received at the time of receipt of the entry into the Promoter’s database and not of transmission by the entrant. The promoter accepts no responsibility for outdated or incorrect contact details by which the entrant cannot be contacted during business hours on the relevant dates.
