City of Playford FormsPort iPad Promotion
Terms and Conditions
1. The promotion is called ‘City of Playford FormsPort IPad Promotion’. Information
on how to enter and prizes form part of these conditions of entry.
2. The promotion period commences 9.00am 1 August 2015 and closes 12.00am 18
September 2015 and includes all entrants who have already registered.
3. Entry is only open to City of Playford residents, business owners/operators and
people who work within Playford, aged 18 and older. City of Playford Councillors
and employees and the promoter’s contractors and agents associated with the
promotion are ineligible to enter.
4. All eligible individuals who register their details to receive City of Playford rates
notice via FormsPort (scan QR code on hardcopy rate notice) by 5.00pm 18
September 2015 are automatically entered into the draw. Only one entry per
person is permitted.
5. Entries not completed in accordance with these conditions will not be eligible to
win. No responsibility is accepted for late, lost or misdirected entries.
6. The Prize draw will take place at Playford Operations Centre, 12 Bishopstone
Road, Davoren Park, 5113 on Wednesday 23 September 2015 at 10am.
7. The winner will be contacted via email within 3 days after the prize is drawn. The
Judges’ decision is final and no correspondence will be entered into.
8. Prize draw – All entrants who register their details to receive City of Playford rates
notice via FormsPort (scan QR code on hardcopy rate notice) will be automatically
in the running to win an Apple iPad Mini, donated to the City of Playford for the
purpose of this competition by Forms Express. This prize will be randomly drawn
and the total value of the prize is approx. $396.00. The Prize is not exchangeable.
9. City of Playford will not be liable for any loss or damage which is suffered (including
but not limited to indirect or consequential loss) or for personal injury which is
suffered or sustained during the course of the promotion, except for any liability
which cannot be excluded by law.
10. City of Playford is not responsible for any problems or technical malfunction of
any telephone network or lines, computer on-line systems, servers, or providers,
computer equipment, software, technical problems or traffic congestion on the
Internet or at any web site, or any combination thereof, including any injury or
damage to participants or any other person’s computer related to or resulting from
participation in or downloading any materials in this competition.
11. If for any reason this promotion is not capable of running as planned including
infection by computer virus, bugs, tampering, unauthorised intervention, fraud,
technical failures of any other causes beyond the control of the promoter, which
corrupt or affect the administration security, fairness integrity or proper conduct of
this promotion, City of Playford reserves the right in its sole discretion to cancel,
terminate, modify or suspend the competition if there is any interruption outside of
City of Playford’s control and to disqualify any individual who interferes with the
entry process.
12. By entering the promotion, unless otherwise advised by the Entrant, each Entrant
consents to the information they submit being entered into a database and City
of Playford may use this information for promotional, marketing and publicity
purposes without any further reference or payment or other compensation to
the Entrant. The Entrant agrees that City of Playford and its related bodies may
send the Entrant commercial electronic messages concerning any information,
promotions, products or services of City of Playford, and its related bodies.
Entrants’ personal information may be disclosed to State and Territory Lottery
Departments and winners’ names published as required under the relevant Lottery
legislation. All personal details of the Entrants provided by entering the promotion
will be stored at the office of City of Playford. A request to access, update or correct
any information should be directed to that office.
