At Know Your Bones we want you to stay active and enjoy what life has to offer!

So we are giving you a chance to self-assess your bone health and go into the draw to win a $250 Westfield Gift Card.

Start by providing your email address – this will ensure you get a pdf copy of your report emailed directly to you AND we will be able to contact you if you are the winner.
Complete the short 5-10min bone health self-assessment to get your report and enter the draw to win.
Once you have completed your assessment print it out to take to your GP to discuss an action plan if required.

 

TERMS AND CONDITIONS
The Promoter is Osteoporosis Australia (ABN 45 098 570 515) located at Level 2, C2.11, 22-36 Mountain Street, Ultimo NSW 2007.

The promotion commences 9:00 am on Thursday 14th February 2019 and closes midnight on Sunday 24th February.

The prize is one $250 Westfield Gift Card expiring January 2022.

To enter the draw, participants must complete the Know Your Bones online assessment at www.knowyourbones.org.au and include their email address during the promotional period.

Only fully completed assessments with the email address will be eligible.

There will only be one winner who will be randomly selected at Level 2, C2.11, 22-36 Mountain Street, Ultimo NSW 2007 by Friday 1st March 2019. The winners will be notified by email by close of business of the next day after the draw.
