Information on how to enter and prizes form part of these Conditions of Entry.
Entry is only open to existing patients and their immediate family members. Employees and immediate families of Gullotta Orthodontics and its agencies associated with this promotion are ineligible.
There is no limit to the number of times an individual may enter the competition.
If you email on behalf of another individual, you warrant that you have sought and received their permission, and that they agree to be bound by these Terms and Conditions and the Privacy Policy of Gullotta Orthodontics.
Gullotta Orthodontics reserves the right to verify the validity of entries and to disqualify any entrant who tampers with the entry process or who submits an entry that is not in accordance with these Conditions of Entry.
Promotion commences 14/02/2011. Entries close 11:59pm EST on Sunday 10/04/2011. Judges decision will be made by Wednesday 27/04/2011 at 129 Nerang Street, Southport QLD. Winner will be notified by email and telephone and the name of winner of the prize valued at over $100 will be published in The Gold Coast Bulletin Tuesday 03/05/2011.
Judges’ decision is final and no correspondence will be entered into.
The competition is a game of skill.
The winner of the competition is not guaranteed that the slogan will be used by Gullotta Orthodontics.
All entries become the property of Gullotta Orthodontics.
Entries which are deemed to not be original will automatically be disqualified from the competition, and no correspondence will be entered into.
If two entries are identical in wording, the entry that is submitted first (as determined by the electronic record of submission) will be deemed to be the original version of that entry, and further identical versions will be invalid for entry in the competition.
The person who authors the winning original slogan may choose from:

All ancillary costs associated with redeeming the prize are the responsibility of the winner. Gullotta Orthodontics shall not be liable for a prize that has been lost, stolen, forged, damaged or tampered with in any way. Redemption of the prize is subject to the terms and conditions specified on any prize literature.
Gullotta Orthodontics may award another winner on 01/06/2011 in order to distribute any prize unclaimed by that date. Winners will be notified by email and telephone and the name of the winner of prizes valued over $100 will be published in The Gold Coast Bulletin on 14/06/2011. Judges’ decision is final and no correspondence will be entered into.
