National Pharmacies Tefal Hamper Competition

To enter, you must complete in full the online entry form available at www.nationalpharmacies.com.au  or send your details and answer to VHcompetition@fsma.com.au by 5 June 2011and tell us in 25 words or less why you visit a National Pharmacies store. One winner will be drawn.

The competition will start on 2 May 2011 and will go through until 5 June 2011

Terms and Conditions:

   1. Entry into this draw is deemed acceptance of and agreement to these rules.
   2. Information on how to enter and the prize form part of the conditions of entry.
   3. Entry into this draw is open to all members residing in South Australia, Victoria and New South Wales, excluding employees and their immediate families of Friendly Society Medical Association Limited and their associated agencies and companies.
   4. To enter, you must complete in full the online entry form available at www.nationalpharmacies.com.au or send your details and answer to VHcompetition@fsma.com.au, by Sunday 5 June 2011 and tell us in 25 words or less why you visit a National Pharmacies store.
   5. Only residents of South Australia, Victoria and New South Wales are eligible to enter the draw.
   6. One winner will be judged based on their answer to the following question: ‘Tell us why you visit a National Pharmacies store'
   7. The prize is a Tefal Hamper valued at $549.90. The Hamper includes a tefal Actifry and VitalCompact Food Processor.
   8. Any additional services and costs involved must be covered by the prize winner.
   9. The winner will be drawn on Friday 10 June 2011 at 11:00am at the promoter’s premises: Level 1, 52 Gawler Place, Adelaide SA 5000.
  10. The winner will be notified by telephone and post on Friday 10 June 2011. A public notice will be included on the National Pharmacies website (www.nationalpharmacies.com.au) on Monday 13 June 2011.
  11. Prizes are not transferable.
  12. Prizes must be redeemed within 3 months of draw date.
  13. The promoter is Friendly Society Medical Association Limited ABN 69 088 347 602 of Level 1, 52 Gawler Place, Adelaide South Australia 5000.
  14. If any prize is not claimed within 3 months draw date, an unclaimed prize draw will be conducted on 26 August 2011 subject to any written directions given under Reg 37 of the Lottery and Gaming Regulations 1993 (SA).
  15. The winner of the unclaimed prize draw will be notified by telephone and post and on 26 August 2011.
  16. The winner’s release the promoter and other organisers, their related bodies corporate, affiliates, officers, agents and employees, from all liabilities, loss and damage of any kind arising at any time out of or in connection with the acceptance of, and participation in, the prize.
  17.  The competition will start Monday 2 May 2011 and will go through until Sunday 5 June 2011.
  18.  All entries remain the property of the promoter.
