National Pharmacies Gift Vouchers Competition 2011

National Pharmacies is offering two lucky members the opportunity to win a $100 National Pharmacies Gift Voucher each to spend at their next pharmacy or optical visit.

Entrants must be a financial member of National Pharmacies, fill out an entry form available online at www.nationalpharmacies.com.au.

The competition will start on Thursday 14th of April and will go through until Monday 30th May 2011.

Terms and conditions:

1. Entry into this draw is deemed acceptance of and agreement to these rules.

2. Information on how to enter and the prize form part of the conditions of entry.

3. Entry into this draw is open all National Pharmacies members, excluding employees and their immediate families of Friendly Society Medical Association Limited and their associated agencies and companies.

4. Only financial National Pharmacies members can enter this competition. To enter, members must fill out an entry form online at www.nationalpharmacies.com.au by midnight on Monday 30th May 2011.

5. Entrants will be put into the draw. Two winners will be drawn. The winner will receive one $100 National Pharmacies Gift Voucher each to spend in store. Valid at all National Pharmacies stores.

6. Any additional services and costs involved (such as transportation)  must be covered by the prize winner.

7. Subject to availability.

8. The winners will be notified by telephone on Monday 30th May 2011 and their name posted on the website.

9. Prize is not transferable and are not redeemable for cash.

10. The promoter is Friendly Society Medical Association Limited ABN 69 088 347 602 of Level 1, 52 Gawler Place, Adelaide South Australia 5000.

11. All entries remain the property of the promoter.

12. Any change in the value of the prize occurring between publishing date and the date the prize is claimed is not the responsibility of the promoter.
