National Pharmacies is offering FOUR members the chance to WIN a $50 David Jones Gift Card each.

Entrants must be a financial member of National Pharmacies, fill out an entry form available online at www.nationalpharmacies.com.au.

The competition will start on Thursday 15th November and will go through until Tuesday 18th December 2012.

Terms and conditions:

1. Entry into this draw is deemed acceptance of and agreement to these rules.

2. Information on how to enter and the prize form part of the conditions of entry.

3. Entry into this draw is open to National Pharmacies members, excluding employees and their immediate families of Friendly Society Medical Association Limited and their associated agencies and companies.

4. Only financial National Pharmacies members can enter this competition. To enter, members must fill out an entry form online at www.nationalpharmacies.com.au

5. Entrants need to complete all the fields on the online form, from which two winners will be randomly selected.

6. Each winning entry will receive a David Jones Gift card valued at $50, with an expiry date of 11/14.

7. The winner will be notified by telephone on Wednesday 19th December and their name posted on the website

8. Prizes are not transferable and are not redeemable for cash.
